Farzin Bahadori

Farzin Bahadori is a dynamic Project Manager with a successful track record in project delivery and thought leadership within the Ontario Health Team (OHT) landscape. Currently serving as the Digital Health Lead at Southlake Community OHT, Farzin has been the driving force behind numerous successful projects, providing visionary leadership and delivering exceptional results. Previous to SC-OHT he played a pivotal role in advancing the establishment of Primary Care Networks within the OHT, program and project delivery benefiting vulnerable communities and COVID-19 response culminating in improvements to healthcare integration. Farzin’s thought leadership was recognized by the appointment of a co-chair position to lead a Regional Working Group on Patient Portals and Navigation and receiving the Staff Excellence in Collaboration award from UofT’s Department of Family and Community Medicine (DFCM) in 2021. With a passion for making a difference in our communities, he continues to drive digital health innovation and improving population health.
