Kathleen McQueen is the Director, Clinical Services Design. She has a Master’s degree in Occupational Therapy and has worked in the home and community healthcare sector for 15 years. She passionately believes in the future of healthcare at home, and is committed to driving the adoption of innovations that deliver better care and positive outcomes in the home and community.

In addition to designing new and operationally sound interdisciplinary clinical models, Kathleen works closely and collaboratively with many teams across Canada – bringing her energy, innovative approach, creativity, and vision to the modernization of healthcare.

Supporting the People Side of Change to Improve Health Outcomes

During the COVID 19 Pandemic, CBI Health quickly mobilized to support the health care system by partnering with LHINs and hospitals to not only expedite discharge home for patients designated ALC but also provide enhanced restorative care focused on patient enablement at home. We will share key elements of our Transition to Restorative Care at Home program including: our restorative care approach, an integrated patient record and virtually connected care teams, patient focused goal attainment and digital enablers. System and patient level outcomes provide evidence that home care plays a pivotal role in supporting patients to age in place at home, while reducing costs and improving the patient and family experience.
